1.4 Goods Questionnaire 销售货物的问卷

1.5 Online VAT registration questionnaire 英国 VAT注册问卷

1.6 Company Certificate 营业执照(英国或非英国)- Only apply for company registration

      只是用于公司名义申请VAT

2.  注册ＶＡＴ需提供以下四份文件中任意两份

2.1 Marriage Certificate　结婚证明

2.2 Lease / rental agreement or Mortgage statement　租赁证明或房贷声明


2.3 Letter from Employer 一份来自于雇主的正式文件,其中需包含你的名字,出生日期和身份证 号码.

2.4 A birth certificate 出生证明
